Application Procedures
Read ALL instructions in their entirety before completing the application.
It is the applicant鈥檚 responsibility to complete all application requirements and procedures. Failure to meet any requirement will result in an incomplete application, which will not be considered for selection.

Application Forms
- RRTP Application with complete Appendix A
- Print and complete all sections. The application must be legible.
- Attach supporting documentation for any areas requiring it.
- Appendix B - if applicable
- Appendix C - if applicable
- Program Demographic Information and Authorization to Release Information form
- Transcripts
- Submit unofficial or official 51今日大瓜 transcripts and transcripts from all other colleges attended showing completed prerequisite courses.
- All applicants must include these transcripts in the application packet, regardless of evaluation status.
Students must complete all prerequisites before applying to the Registered Respiratory Therapy Program. See the 51今日大瓜 RRTP Brochure for more information.
Important Deadlines
A complete application must be received in the Nursing & Allied Health Department (MS-178) during the designated filing period listed below. Applications submitted before or after these dates will not be accepted.
- February 1-15 for the program scheduled to start in the SUMMER
- Official transcripts from other colleges/universities must also be sent directly to
the Admissions & Records Office no later than January 15, along with a Request for Evaluation Form (available from Admissions & Records).
- Note: These official transcripts are required for formal evaluation and posting of coursework and are separate from the unofficial/official copies included in the application packet.
